These evalautions can be set up for individuals or for your business. An individual assessment of job performance many times turns into a total business or organization evaluation. We have found this may also go the other way, starting with a business evaluation and lending itself to individuals assessements.
Example 360° Assessment for Administrator or Leader
You are being asked to assess job behaviors for ________. Please read each statement carefully and use the scale listed to indicate your response.
We urge you to be completely honest in your answers. There are no "right" or "wrong" answers. Effective feedback comes from your straightforward reaction to the particular behavior in each statement. Please read each statement carefully and choose the rating that best reflects your observations. If you feel that you do not have enough information to respond to an item, or if the item does not apply to this person’s role, please mark Don’t Know or Not Observed.
When selecting your response to each statement:

Be realistic about the extent to which this person actually engages in the behavior.

Be as honest and as accurate as you can be.

DO NOT answer in terms of how you would like to see this person behave or in terms of how you think this person should behave.

DO answer in terms of how this person typically behaves on most days, on most projects, and with most people.
At the end of the survey, there are open-ended questions with spaces for comments. It is important that you answer these questions with your best thinking. Written comments will be included as part of a feedback report to be used for development purposes and will be presented as verbatim.
Please note that your feedback will not be identified back to you. With the exception of responses from a direct manager, all responses will be combined into rater categories and presented as a group. Depending on what you write, your comments can identify you; however the comment responses are not connected to each other or to your other responses in the report. Identifying yourself in one comment will not identify all of your feedback.
